Why we ask
We want everyone to be able to use our service. The Equality Act 2010 says we must make changes (Reasonable Adjustments) for disabled people and people with long‑term conditions. You only need to tell us what you feel comfortable sharing.
Medical Conditions
Does anyone in the household have any medical conditions? If you would like to share what they are, we can record them for each household member.
Disability Status
Would you consider yourself or anyone in your household to be disabled? This means having a medical or mental health condition that affects day to day life and has lasted more that 12 months.
Reasonable Adjustments
We can record reasonable adjustments or support needs to make sure our staff and contractors deliver services in a way that works for you. Are there any adjustments you would like us to note? This could include things like getting information in a different format, large print, someone to help with communication or more time to answer the door.
